As far as new posts go, I can't speak for everyone but I do check my friends feed pretty regularly so if a post is made for me, I should see it. If a day or two goes by and I haven't replied and you don't think I know about it, just PM me, I guess. It could happen that I miss things, especially with multiple accounts going. Though I did just go and turn pingbacks on for this account so I guess I'd have to see how/if that works.
But as far as replying to threads within posts, if you reply to the comment rather than the post, the person will get a notification. If someone is replying to your thread in a post they made with a whole new comment, you're not going to get a notification because they're not directly replying to you. So unless you remember how many comments the post had it might go unnoticed that there was even a reply unless it's made directly to the comment rather than the post. Does that make sense? It's early and my brain isn't awake yet.
Sometimes LJ has issues with that (like any site) but since I've been writing here again it's been pretty good about both email notifs and notifs on the sidebar of the friends feed.
